Step 1: Download Forms
Download necessary forms and complete at home.
Consent Form
Print and complete at home. Please ATTACH completed form to your registration.
Health Form
Print and complete at home. Please ATTACH a copy of your insurance card and camper's vaccination records.
Scholarships
If the cost of camp is a concern, partial scholarships are available. Download an application or submit ONLINE below by 6/1/2025.
Step 2: Submit Forms
You have three options to submit forms:
1. Bring to Camp
Exception: Wakefield forms are due online by 6/1.

Completed forms can be ATTACHED (up to 25mb) to the REGISTRATION toward end of form, use camper's NAME in file name.

You have three options to pay your remaining balance (after $50 deposit):

Bring your balance due to camp (checks preferred) made out to Wisconsin Congregational Association.

Submit payment for your balance due via PayPal below. Please enter your amount due under 'price per item'
(if paying for more than one camper, select quantity):
